TizbiFormsT Requirements TizbiForms Optional Additions
Tablet PC: The TizbiForms customer is responsible for purchasing or providing an approved tablet PC with the required software. Tizbi will configure and purchase a tablet on the customer's behalf for the full cost of the tablet plus a 10% service fee. Details can be found in the Tablet PC Guide.
Setup Fee: The new customer setup fee is $300. This fee includes configuration of the customer's organization, the creation of all users and all necessary configurations of security and rights to forms and departments. Users will be assigned to departments within the organization.
Existing Form Customization: Tizbi, Inc. is dedicated to increasing its form library by developing forms that are standardized and sanctioned by large organizations. These forms can be reused by multiple customers. Only minor changes are required to configure a form that exists in the Tizbi library. The fee to configure an existing standard form is $250. If customization of an existing form is required, the standard customization prices for adding fields, filters, and grid items apply.
Custom Form Development: Custom form development is required when the form that is being automated does not exist in the Tizbi library. Custom form development pricing includes electronic form creation (includes filter and grid creation with 10 fields in each). Tizbi regularly runs special deals and discounts on custom form creation, contact us to get an exact quote.

Wireless Connectivity: The TizbiForms customer is responsible for purchasing and maintaining all wireless hardware and service. Details about available wireless devices and services can be found in the Wireless Access Guide. A cellular wireless card and service is not required if the user submits all forms in the range of an 802.x wireless network that has access to the internet. A broadband wireless service is recommended for users who will be working in a mobile environment.
Support and Training: E-mail and phone support and training are included in the monthly fee. WebEx support is also included in the monthly fee. WebEx training and on-site support and training are available at $75 per hour, plus travel expenses for on-site support and training.
Additional Features: Customers are given an optional data editing feature. This feature gives users the ability to edit data in a web interface after the form has been sent from the tablet. The optional editing feature fee for a standard form is $350. There is a $75 fee for each additional 50 fields. If customers would like to add other optional features, contact us and we will evaluate if it can be configured into the software, and at what price.
